Abstract:
Background: In older adults, episodes of dependency, somatic symptom escalation, and increasing medication focus are often understood biomedically. They may also indicate overloaded state regulation under loss, pain, shame, and loneliness. Method: This conceptual paper links psychotherapy with older adults to a neurobiopsychosocial view of bodily alarm, protective patterns, psychotherapeutic workability, and social context. PAD-S (Perceive–Assess–Dose–Safeguard) is adapted for psychotherapy with older adults. Cognitive perceptual disruption (CPD) is discussed as a potential indicator of cognitive-perceptual overload; COPEDS (COgnitive PErceptual Disruption Screening) is presented as an adjunctive self-report instrument. Results: In selected cases, stress-related regression, pain exacerbation, and opioid focus may be associated with the regulation of alarm and separation distress. PAD-S supports medical safety checks, graded emotional-bodily activation, and interprofessional coordination. Conclusion: The framework offers observable, team-compatible heuristics for the next safe step but does not replace medical or gerontopsychiatric assessment.
